The CarePartner Program: Diabetes

With leadership from CMCD affiliate James Aikens, PhD, we are conducting a randomized trial funded by the National Institute of Diabetes and Digestive and Kidney Diseases in order to evaluate the effectiveness of the CarePartner Program for patients with type 2 diabetes. Patients randomized into the intervention arm receive automated telemonitoring and self-management assistance calls for one year. At baseline, patients identify an informal caregiver or CarePartner who receives e-mailed summaries of the telemonitoring data along with guidance on helping the patient address any reported problems. If medically urgent problems are identified during the automated calls, the patient’s clinician is alerted automatically. Patients randomized to the control arm receive usual care. Outcomes measured at six months and one year include blood glucose control, blood pressure, and self-management behaviors.
